    Understanding Stress In The Operating Room: A Step Toward Improving The Work Environment

    Job-related stress is an important factor predicting staff satisfaction and position turnover among nursing staff, particularly in the operating room. The purpose of this study was to examine the perceived amount of stress elicited by events in the perioperative environment, the frequency of those events, and the impact of those events on the perceived stress of operating room nurses (ORNs) and operating room technologists (ORTs). The Survey on Stress in the OR instrument, which was used to query the subjects, exhibited high internal consistency of all items. The findings indicated that the ORNs and the ORTs exhibited remarkable similarities between stressful events perceived as high and low impact. The two groups agreed that the highest impact stressful event was pressure to work more quickly. Using the results of this study, OR administrators may be able to redesign the OR environment to minimize the impact of stressful events and thereby improve job satisfaction and minimize nursing staff turnover

Unfortunately, attending meetings regularly is often a challenge for nurses, and relatively few have the knowledge and expertise to adequately critique research articles. One way to bridge the limitations of accessibility and limited research expertise of journal club members is to establish a virtual journal club.     Psychometric Evaluation of an Instrument to Measure Hispanic Mothers’ Normative Beliefs, Intentions, Past Experience and Past Behavior Related to the Discussion of Sex-Related Topics with their Adolescent Daughters

    Aim: The aim of this project was to describe the psychometric properties of a recently developed instrument that measures Hispanic mother normative beliefs, intentions, past experience, and past behavior regarding the Discussion of Sex-Related Topics (DSRTs) with their adolescent daughters. Background: The DSRTs between Hispanic mothers and their adolescent daughters is important because this interaction has been found to be a protective factor against the daughter’s risk for STDs and unintended pregnancy. Hispanic mother talk less with their daughters about sex-related topics (SRTs) than other ethnic groups. The Rodriguez Normative Belief Instrument (RNBI) has been established to measure normative beliefs, normative beliefs, past behavior, and past experience regarding Hispanic mothers’ DSRTs with their adolescent daughters. Method: The 44-item RNBI was developed based on the Theory of Planned Behavior and the Parent-Based Expansion Theory of Planned Behavior. A convenience sample of 119 Hispanic mothers of adolescent females grades 6 through 8 from two Midwestern Catholic Middle Schools was enrolled completed the instrument. Paper and pen instruments were administered in the language of the participant’s choice, English or Spanish. Results: The entire RNBI and each of the four subscales demonstrated acceptable internal consistency (Cronbach’s alpha = .70). The overall reliability of the RNBI was .82 with the normative beliefs subscale of .85, intentions .88, past behavior .89, and past experience .88. Confirmatory factor analysis validated the four subscales. Conclusion: Psychometric analyses indicated that the RNBI demonstrates acceptable validity and reliability for this sample

    Answering Wicked Questions: Dealing With Opposing Truths as a Nursing Associate Professor

    Nursing associate professors frequently are confronted with increasing responsibilities and fewer resources. These challenges commonly contribute to declines in job satisfaction and may result in departing academe. This article addresses these challenges by providing answers to four common “wicked questions” experienced by nursing associate professors: (a) How do I decline a request from a supervisor to take on additional responsibilities while continuing to support the mission of the school and advance my own scholarly productivity? (b) How do I handle the workload of multiple doctoral students with a variety of content areas that are different from my own and maintain my own level of productivity? (c) How do I handle expectations for more service, and leadership for the school, university, and professional organizations, yet the teaching and research responsibilities have not changed or have increased? and (d) What are some additional tips to being a more productive nursing associate professor

    Topical Menthol, Ice, Peripheral Blood Flow, and Perceived Discomfort

    Context : Injury management commonly includes decreasing arterial blood flow to the affected site in an attempt to reduce microvascular blood flow and edema and limit the induction of inflammation. Applied separately, ice and menthol gel decrease arterial blood flow, but the combined effects of ice and menthol gel on arterial blood flow are unknown. Objectives : To compare radial artery blood flow, arterial diameter, and perceived discomfort before and after the application of 1 of 4 treatment conditions. Design : Experimental crossover design. Setting : Clinical laboratory. Participants or Other Participants : Ten healthy men, 9 healthy women (mean age = 25.68 years, mean height = 1.73 m, mean weight = 76.73 kg). Intervention(s) : Four treatment conditions were randomly applied for 20 minutes to the right forearm of participants on 4 different days separated by at least 24 hours: (1) 3.5 mL menthol gel, (2) 0.5 kg of crushed ice, (3) 3.5 mL of menthol gel and 0.5 kg of crushed ice, or (4) no treatment (control). Main Outcome Measure(s) : Using high-resolution ultrasound, we measured right radial artery diameter (cm) and blood flow (mL/min) every 5 minutes for 20 minutes after the treatment was applied. Discomfort with the treatment was documented using a 1-to-10 intensity scale. Results : Radial artery blood flow decreased (P \u3c .05) from baseline in the ice (−20% to −24%), menthol (−17% to −24%), and ice and menthol (−36% to −39%) treatments but not in the control (3% to 9%) at 5, 10, and 15 minutes. At 20 minutes after baseline, only the ice (−27%) and combined ice and menthol (−38%) treatments exhibited reductions in blood flow (P \u3c .05). Discomfort was less with menthol than with the ice treatment at 5, 10, and 20 minutes after application (P \u3c .05). Arterial diameter and heart rate did not change. Conclusions : The application of 3.5 mL of menthol was similar to the application of 0.5 kg of crushed ice in reducing peripheral blood flood. Combining crushed ice with menthol appeared to have an additive effect on reducing blood flow

    Prediction of VO\u3csub\u3e2\u3c/sub\u3e Peak Using Sub-Maximum Bench Step Test in Children

    The purpose of this study was to develop a valid prediction of maximal oxygen uptake from data collected during a submaximum bench stepping test among children ages 8-12 years. Twentyseven active subjects (16 male and 11 female), weight 36.1 kg, height 144.4 cm and VO2 47.4 ± 7.9 ml/kg/min participated. Subjects completed a maximal oxygen consumption test with analysis of expired air and a submaximal bench stepping test. A formula to predict VO2max was developed from height, resting heart rate and heart rate response during the submaximum bench stepping test. This formula accounted for 71% of the variability in maximal oxygen consumption and is the first step in verifying the validity of the submaximum bench stepping test to predict VO2max. VO2max = -2.354 + (Height in cm * 0.065) + (Resting Heart Rate * 0.008) + (Step Test Average Heart Rate as a Percentage of Resting Heart Rate * -0.870

    The Impact of Simulation Sequencing on Perceived Clinical Decision Making

    An emerging nursing education trend is to utilize simulated learning experiences as a means to optimize competency and decision making skills. The purpose of this study was to examine differences in students\u27 perception of clinical decision making and clinical decision making-related self-confidence and anxiety based on the sequence (order) in which they participated in a block of simulated versus hospital-based learning experiences. A quasi-experimental crossover design was used. Between and within group differences were found relative to self-confidence with the decision making process. When comparing groups, at baseline the simulation followed by hospital group had significantly higher self-confidence scores, however, at 14-weeks both groups were not significantly different. Significant within group differences were found in the simulation followed by hospital group only, demonstrating a significant decrease in clinical decision making related anxiety across the semester. Finally, there were no significant difference in; perceived clinical decision making within or between the groups at the two measurement points. Preliminary findings suggest that simulated learning experiences can be offered with alternating sequences without impacting the process, anxiety or confidence with clinical decision making. This study provides beginning evidence to guide curriculum development and allow flexibility based on student needs and available resources

    A Comparison of Two Formulas of Topical Menthol on Vascular Responses and Perceived Intensity Prior to and following a Bout of Maximum Voluntary Muscular Contractions (MVMCs)

    The purpose of this study was to compare the vascular responses in the brachial artery and perceived intensity of two different formulas of topical menthol gels prior to and following a bout of maximum voluntary muscular contractions (MVMCs). 18 adults completed the same protocol on different days using blinded topical menthol gels (Old Formula and New Formula). Heart rate, brachial artery blood flow (ml/min), vessel diameter and reported intensity of sensation were measured at baseline (T1), at 5 min after application of the gel to the upper arm (T2), and immediately following five MVMCs hand grips (T3). The New Formula exhibited a significant decline in blood flow (−22.6%) between T1 and T2 which was not different than the nonsignificant declines under the Old Formula 1 (−21.8%). Both formulas resulted in a significant increase in perceived intensity of sensation between T1 and T2. Blood flow increased significantly with the New Formula (488%) between T2 and T3 and nonsignificantly with the Old Formula (355%)
